We accept cash, debit cards, and credit cards. We do not bill health insurance. We offer payment plans, prepaid packages, and CareCredit, and we bill auto insurance (MedPay) for car accident injuries. You will get your care plan and financial options at your first visit.

A parent or legal guardian must attend, even if the student is 18. Complete the student/parent history and consent sections of the school physical form before arrival; the provider completes the examination section. Bring medication and injury information and wear athletic clothing and shoes with a back. Physicals are by appointment only: $35, with a cash discount bringing the fee to $25.
